About this project
SNC is a mobile healthcare platform that connects patients with doctors and medical care, secured through blockchain technology. The app covers the full care journey — from patient onboarding and symptom assessment to appointment booking, treatment tracking, and care sponsorship — serving both patients seeking care and donors who want to fund it.
Problem Statement
In many underserved regions, people face serious barriers to accessing healthcare — from financial constraints and lack of trusted medical infrastructure, to having no unified system to manage their health identity or care journey. Without a reliable way to connect patients with verified doctors, track treatments, or channel charitable funding transparently, many people are left without the care they need.
Solution
SNC brings the entire healthcare experience into one blockchain-secured mobile app. Patients build a verified health profile, go through a guided symptom assessment, discover and book doctors, and track their care from start to finish. For those who can't afford treatment, a community donation model allows sponsors to fund specific care requests with full transparency. Post-care instructions and medical history are also stored in-app, giving patients a continuous, trustworthy record of their health.
